FERPA Forms

The following forms are available online for the convenience of students:

All forms must be returned to the Center for Student Success and Coaching as they are the official documents required for submission. Handwritten letters to release or withhold information are not accepted as official requests. Requests can be submitted in person or notarized.

• Authorization to Withhold the Release of Directory Information
  Use this form to withhold the releasing of Directory Information (See above for definition) to any third party. Must be filed each academic year.


• FERPA Authorization Form - Consent to Release Educational Records
  Use this form to authorize the release of your educational records to designated third parties under the Family Educational Rights and Privacy Act (FERPA).
